Ordering Information for Colleges

To order ESS materials, including surveys, normative data reports, specimen set, User's Guide, or sample item catalog:

  1. Download and print out the ESS Postsecondary Order Form (PDF; 2 pages, 107KB)
     
  2. Complete the form, following the directions provided, and mail or fax it to:

  3. Provide payment as directed on the form. We also accept payments via MasterCard or Visa.
 

For processing of surveys:

  1. Download and print out the ESS Postsecondary Data Form (PDF; 2 pages, 68KB)
  1. Respond to the questions on the Survey of Academic Advising (third page of the data form) only if you are returning Survey of Academic Advising instruments for scanning.
  1. Complete the form, following the directions provided, and enclose it with your surveys when you return them for scanning.
