Examinee Records Modification Request

If you have taken ACT WorkKeys tests but received score reports with incorrect demographic information, then complete and submit this form to request an update of your records. 

Note: If a test was taken under another examinee’s account, the site administrator should file an irregularity report and the examinee will have to retest.
